چکیده

Very low birth weight (VLBW) infants require specialized care in neonatal intensive care units. In the United States (U.S.), such infants frequently are transferred between hospitals. Although these neonatal transfer networks are important both economically and in terms of infant morbidity and mortality, the national-level pattern of neonatal transfers is largely unknown. Using data on 44,753 infants, 2,122 hospitals, and 9,722 inter-hospital infant transfers from 2015, we performed the largest analysis to date on the inter-hospital transfer network for VLBW infants in the U.S. We found that transfers are organized around regional communities, that, despite being largely within state boundaries, most often contain at least two hospitals in different states. Applying a spectral measure of hierarchy, we find a positive correlation between our measure and information about infants in the detected communities. Our results have a practical implication of characterizing neonatal care in the U.S. and may broadly apply to the role of hierarchical forces in organizing complex adaptive systems. Although very low birth weight (VLBW) infants, weighing less than 1,500 grams at birth, accounted for only 1.4% of all births in the United States (U.S.) in 2015, they accounted for 52% of all infant deaths [1]. These extremely fragile infants require specialized care in a neonatal intensive care unit (NICU). The creation of regional systems for perinatal care, first proposed in 1975 [2], envisioned three levels of care including NICUs, in which the hospital where a mother or infant received care was determined by the complexity of the patient?s needs. The goal was to improve outcomes for highrisk pregnant women and VLBW infants by ensuring access to high-quality, economically-efficient care for all mothers and their newborns [2]. 1 ar X iv :1 80 2. 02 85 5v 1 [ ph ys ic s. so cph ] 2 6 Ja n 20 18

Metabolic Bone Disease in Very Low-Birth-Weight Neonates

Background: Metabolic bone diseases (MBD), including rickets and osteopenia, are important neonatal complications among preterm infants. This study aimed to determine the prevalence and risk factors of MBD in neonates with very low birth weight (VLBW). Methods: This prospective study was conducted on VLBW infants from January 2012 to July 2013. Inclusion criteria were birth weight of ≤1500 g a...

Frequency of Thyroid Function Disorders among a Population of Very-Low-Birth-Weight Premature Infants

Background: Thyroid function disorders, particularly congenital hypothyroidism (CHT), are important endocrine dysfunctions associated with permanent morbidities. CHT is more prevalent among preterm low-birth-weight neonates compared to term infants with normal weight. Methods: This prospective cohort study was conducted on 126 very-low-birth-weight (VLBW) neonates referred to the neonatal inten...

Comparison of Continuous and Intermittent Feeding Methods in Low Birth Weight Infants

Objective About 1 % of infants are of very low birth weight. However, they comprise about 50% of infant mortality. We compare the effects of continuous versus intermittent feeding on physical growth, gastrointestinal tolerance and macronutrient retention in low birth weight infants (Methods A prospective randomized trial clinical trial was performed from 2004-2005. Very low birth weight neonate...

Effects of Probiotic Lactobacillus Reuteri (DSM 17938) on the Incidence of Necrotizing Enterocolitis in Very Low Birth Weight Premature Infants

Background: Feeding intolerance is a common problem among premature infants. There is limited information on the safety and effects of oral probiotic supplements, especially products containing Lactobacillus reuteri, and the incidence of necrotizing enterocolitis (NEC) in low birth weight preterm infants. This study aimed to evaluate the effects of Lactobacillus reuteri on the gastrointestinal ...

FREQUENCY OF HYPOCALCEMIA IN LOW BIRTH WEIGHT INFANTS AT HOSPITALS IN KASHAN IN 1997

Considering the fact that early hypocalcemia is the most common type of neonatal hypocalcemia which manifests during the first few days of Iife (72 hours) and that hypocalcemia in low birth weight neonates is very common, this descriptive study on hypocalcemia was conducted to determine the frequency of hypocalcemia in low birth weight neonates in the city of Kashan in the first 9 months of...
